Welcome to the website of Amanda Doyle MD PLLC (“Practice,” “we,” “our,” or “us”). By accessing or using this website (the “Site”), you agree to be bound by these Terms & Conditions (“Terms”). If you do not agree, please do not use this Site.
1. PURPOSE OF THIS WEBSITE – NO MEDICAL ADVICE
This Site provides general educational and informational content only. It does not provide medical advice, diagnosis, or treatment and does not replace consultation with a licensed medical professional. Use of this Site does not create a physician-patient relationship.

2. NO PHYSICIAN-PATIENT RELATIONSHIP
No physician-patient relationship is formed through use of this Site, submission of online forms, appointment requests, emails, or messages. A relationship is established only after you are formally accepted as a patient and evaluated by the Practice.
3. HIPAA & PRIVACY
Your use of this Site is governed by our Privacy Notice and Website Privacy Policy, which describes how personal information and PHI are handled. Do not submit sensitive medical information through general website forms.
4. TELEHEALTH TERMS OF SERVICE
If telehealth services are offered:
  • Telehealth is voluntary and subject to separate informed consent
  • Telehealth may involve technical limitations and privacy risks
  • Telehealth may not be appropriate for all conditions
  • The Practice may determine that in-person care is required
  • Telehealth services are governed by applicable federal and New York State laws
Use of telehealth services constitutes acceptance of these limitations and risks.
5. ONLINE BOOKING, APPOINTMENTS & CANCELLATION POLICY
If online scheduling is available:
  • Appointment requests are not guaranteed until confirmed
  • You agree to provide accurate and complete information
  • Late cancellations or no-shows may be subject to fees
  • Fees and policies are posted separately and may change
The Practice reserves the right to refuse or cancel appointments.
6. FINANCIAL POLICY, REFUNDS & PAYMENTS
If you purchase services, deposits, or gift cards online:
  • Payments are processed by secure third-party processors such as Jane, a secure-PCI compliant integrated payment processor
  • The Practice does not store full payment card information
  • Refund eligibility is governed by the Practice’s financial policies
  • Cosmetic services are non-refundable once rendered
  • Pricing and services may change without notice and any quoted prices are good for 3 months from the date of quote 
Chargebacks or payment disputes may result in suspension of access to online services.
7. NO SURPRISES ACT – GOOD FAITH ESTIMATE
If you are uninsured or self-pay, you have the right to receive a Good Faith Estimate of expected charges for non-emergency medical services under the federal No Surprises Act.
If your final bill is substantially higher than your estimate, you may dispute the bill through the U.S. Department of Health and Human Services dispute resolution process.
8. USER RESPONSIBILITIES & ACCEPTABLE USE
You agree not to:
  • Use the Site for unlawful purposes
  • Attempt unauthorized access
  • Transmit harmful code
  • Impersonate others
  • Interfere with Site operations
  • Collect data from the Site without permission
Violation may result in termination of access along with any other available legal remedies. 
9. INTELLECTUAL PROPERTY
All content is owned by or licensed to the Practice and protected by intellectual property laws. Personal, non-commercial use only. No reproduction without written consent.
